Oak Grove Center offers three specialized Wraparound programs grounded in the core belief that youth and families do better when they are connected to their schools, communities, and each other. Guided by Wraparound’s 10 Principles, each program delivers team-based, culturally responsive, and highly individualized support for children, youth, and caregivers navigating emotional, behavioral, or mental health challenges. Our family-centered approach ensures that every family’s experience in Wraparound reflects their unique strengths, needs, and goals of the family.
Since 2010, Oak Grove has worked alongside families to build sustainable support systems through a flexible blend of in-home, community-based, and virtual services. Whether connected to Wraparound through Special Education, Managed Care, or Post-Adoptions, our Wraparound programs are designed to help families thrive at home, in school, and in life.
Comprehensive Mental Health Support for Kaiser-Connected Families
Oak Grove’s Managed Care Wraparound Program, in partnership with Kaiser Permanente, provides holistic, family-centered services to youth with complex mental, developmental, and behavioral health needs. Wraparound services are designed to stabilize the home environment by providing intensive therapeutic and behavioral health interventions utilizing a trauma-informed approach designed to increase caregiver confidence and overall family well-being.
Your Managed Care Wraparound Team includes a Therapist, a Behavioral Specialist, a Facilitator, and a Parent Partner, all working together to support the long-term goals of the family through weekly in-home or virtual sessions.

Supporting School Success Through School Districts and Family Partnerships
The School District Funded Wraparound Program supports Special Education students identified for services through the student’s Educationally Related Mental Health Services (ERMHS) Assessment conducted by the child’s Individualized Education Plan (IEP) Team. Oak Grove Center partners with school districts to provide intensive, team-based services focused on school success and family stability.
The strengths-based and culturally responsive team engages children, youth, families, and educational staff in collaborative planning and problem-solving to assist the student in achieving academic success while improving relationships with their family and peers. Services are delivered on an individualized basis and may occur in the home, in school, and in the community, with a flexible blend of in-person and virtual options.

Reimagining Post-Adoption Support Through Wraparound
Oak Grove’s Adoptions Assistance Wraparound Program is a trauma-informed, family-driven program designed specifically for adoptive families navigating the unique joys and complexities that can arise post-adoption. Funded through the Adoption Assistance Program (AAP), this model provides individualized support to children and youth adopted through the child welfare system and their caregivers.
The goal is to strengthen family cohesion, promote healing, and increase long-term stability by addressing emotional, behavioral, and relational challenges that may emerge post-adoption. The Wraparound process creates a supportive team around the family, offering coordinated care, coaching, and mental health resources.
