The Wraparound Program utilizes a team-based and intensive approach to meeting the needs of Youth and their families while being individualized, strengths-based, and culturally competent. Wraparound seeks to achieve positive outcomes by utilizing a structured, creative, and collaborative process of planning for the Youth’s success. Oak Grove  provides linkages to additional resources and services for the family while putting an emphasis on building the family’s social support network. The single most important outcome of the Wraparound approach is a Youth thriving at home and in school while supported by several community services and informal supports.

Oak Grove Center offers three specialized Wraparound programs grounded in the core belief that youth and families do better when they are connected to their schools, communities, and each other. Guided by Wraparound’s 10 Principles, each program delivers team-based, culturally responsive, and highly individualized support for children, youth, and caregivers navigating emotional, behavioral, or mental health challenges. Our family-centered approach ensures that every family’s experience in Wraparound reflects their unique strengths, needs, and goals of the family.

Since 2010, Oak Grove has worked alongside families to build sustainable support systems through a flexible blend of in-home, community-based, and virtual services. Whether connected to Wraparound through Special Education, Managed Care, or Post-Adoptions, our Wraparound programs are designed to help families thrive at home, in school, and in life.

Managed Care Wraparound Program

Comprehensive Mental Health Support for Kaiser-Connected Families

Oak Grove’s Managed Care Wraparound Program, in partnership with Kaiser Permanente, provides holistic, family-centered services to youth with complex mental, developmental, and behavioral health needs. Wraparound services are designed to stabilize the home environment by providing intensive therapeutic and behavioral health interventions utilizing a trauma-informed approach designed to increase caregiver confidence and overall family well-being.

Your Managed Care Wraparound Team includes a Therapist, a Behavioral Specialist, a Facilitator, and a Parent Partner, all working together to support the long-term goals of the family through weekly in-home or virtual sessions.

Services may include:

  • Weekly Family Team Meetings (virtual or in-home)
  • Parent Partner coaching and mentorship
  • Behavioral Support for children and youth at home and in the community
  • Individual and/or Family Therapy with licensed or Associate Therapists Psycho-Education and Coping Skills Training
  •  Oak Grove focuses on whole-family healing and empowerment through a connected, compassionate approach.

Educationally Related Mental Health Services (ERMHS) Wraparound Program

Supporting School Success Through School Districts and Family Partnerships

The  School District Funded Wraparound Program supports Special Education students identified for services through the student’s Educationally Related Mental Health Services (ERMHS) Assessment conducted by the child’s Individualized Education Plan (IEP) Team. Oak Grove Center partners with school districts to provide intensive, team-based services focused on school success and family stability.

The strengths-based and culturally responsive team engages children, youth, families, and educational staff in collaborative planning and problem-solving to assist the student in achieving academic success while improving relationships with their family and peers. Services are delivered on an individualized basis and may occur in the home, in school, and in the community, with a flexible blend of in-person and virtual options.

Services may include:

  • Weekly Family Team Meetings to align goals and strategies
  • One-on-one Parent Partner support from peers with lived experience
  • Behavioral Specialist support for students to build social skills and coping strategies
  • Weekly School team collaboration

All services are strength-based, trauma-informed, and guided by Individualized Care Plans.

Adoption Assistance Wraparound Program

Reimagining Post-Adoption Support Through Wraparound

Oak Grove’s Adoptions Assistance Wraparound Program is a trauma-informed, family-driven program designed specifically for adoptive families navigating the unique joys and complexities that can arise post-adoption. Funded through the Adoption Assistance Program (AAP), this model provides individualized support to children and youth adopted through the child welfare system and their caregivers.

The goal is to strengthen family cohesion, promote healing, and increase long-term stability by addressing emotional, behavioral, and relational challenges that may emerge post-adoption. The Wraparound process creates a supportive team around the family, offering coordinated care, coaching, and mental health resources.

Services may include:

  • Weekly Family Team Meetings to align goals and strategies
  • Parent Partner mentorship from peers with lived experience
  • Behavioral Specialist Sessions to help youth navigate triggers and build coping skills
  • Therapeutic support tailored to adoption-related challenges (e.g., identity, attachment, loss)

Families are never alone; the Team walks alongside you with respect, compassion, and a deep understanding of the adoptive journey.
